Shared Care Agreements

Shared care agreements outline the specialist/GP/patient responsibilites when shared care drugs are initiated, and the patient stabilised on the drug, by the specialist and then the patient's GP is asked to continue prescribing.

The available agreements are for the following  therapy drug areas:

Cancer; Renal; Paediatrics; Mental Health; Epilepsy; Neurology and Miscellaneous drugs       

In the Admin documents folder below you will find a template letter for the specialist to send to the GP when requesting shared care. The specialist should provide a copy of the shared care agreement together with the request letter.
